Transition Planning

When we think of Transition Services, we focus on the ages of 17-24.  Your child will soon be leaving the educational system and entering adult life.  Your family is also staring down the pivotal age of 18, when your child becomes a legal adult.  There are key decisions and specific action steps that need to be taken in regards to Guardianship, applying for government benefits, and answering critical questions:

Is your child 17 or 18-years-old?

Do you have a guardianship in place?

Do you qualify for supplemental security income?

Are you accessing Medicaid disability services?

Do you have a written plan for your child’s future?
